The bed bug may be just as dangerous as its sinister cousin, the triatomine, or kissing bug. A new study from Penn Medicine researchers in the Center for Clinical Epidemiology and Biostatistics demonstrated that bed bugs, like the triatomines, can transmit Trypanosoma cruzi, the parasite that causes Chagas disease, one of the most prevalent and deadly diseases in the Americas.

The role of the bloodsucking triatomine bugs as vectors of Chagas diseasewhich affects 6 to 8 million worldwide, mostly in Latin America, and kills about 50,000 a yearhas long been recognized. The insects infect people not through their bite but feces, which they deposit on their sleeping host, often around the face, after feeding. Bed bugs, on the other hand, are usually considered disease-free nuisances whose victims are left with only itchy welts from bites and sleepless nights.

In a study published online this week in the American Journal of Tropical Medicine and Hygiene, senior author Michael Z. Levy, PhD, assistant professor in the department of Biostatistics and Epidemiology at the University of Pennsylvanias Perelman School of Medicine, and researchers at the Universidad Peruana Cayetano Heredia in Peru conducted a series of laboratory experiments that demonstrated bi-directional transmission of T. cruzi between mice and bed bugs.

In the first experiment run at the Zoonotic Disease Research Center in Arequipa, Peru, the researchers exposed 10 mice infected with the parasite to 20 uninfected bed bugs every three days for a month. Of about 2,000 bed bugs used in the experiment, the majority acquired T. cruzi after feeding on the mice. In a separate experiment to test transmission from bug to mouse, they found that 9 out of 12 (75 percent) uninfected mice acquired the parasite after each one lived for 30 days with 20 infected bed bugs.

In a third experiment, investigators succeeded in infecting mice by placing feces of infected bed bugs on the animals skin that had either been inflamed by bed bug bites, or scraped with a needle. Four out of 10 mice (40 percent) acquired the parasite by this manner; 1 out of 5 (20 percent) were infected when the skin was broken by the insects bites only. A final experiment performed at the Penn bed bug lab in Philadelphia demonstrated that bed bugs, like triatomines, defecate when they feed.

Weve shown that the bed bug can acquire and transmit the parasite. Our next step is to determine whether they are, or will become, an important player in the epidemiology of Chagas disease, Levy said. There are some reasons to worrybed bugs have more frequent contact with people than kissing bugs, and there are more of them in infested houses, giving them ample opportunity to transmit the parasite. But perhaps there is something important we dont yet understand about them that mitigates the threat.

T. cruzi is also especially at home in the guts of bed bugs. Ive never seen so many parasites in an insect, said Renzo Salazar, a biologist at the Universidad Peruana Cayetano Heredia and co-author on the study. I expected a scenario with very low infection, but we found many parasitesthey really replicate well in the gut of the bed bugs.

Wicked Cousins

Bed bugs and kissing bugs are distant cousins but share many striking similarities. Both insects hide in household cracks and crevices waiting for nightfall and the opportunity to feed on sleeping hosts. They are from the same order of insects (Hemiptera) and both only feed on blood. (One main difference is their size: kissing bugs are five times as big as a bed bug). With so much in common, it seemed logical to the authors that the kissing bugs most infamous trait, the transmission of T. cruzi, is also shared by the bed bug.

Other investigators have shared this suspicion. In 1912, just three years after Carlos Chagas described the transmission of the disease by kissing bugs, French parasitologist mile Brumpt recounted that he had infected almost 100 bed bugs exposed to an infectious mouse, and then used them to infect two healthy mice. Decades later an Argentine group replicated his work. These experiments, largely ignored during the recent bed bug resurgence, missed one key point.

Bedbugs are becoming a problem everywhere. Just because a hotel or any other type of accommodation has had them in the past doesn't mean they still do, and just because they didn't have them one month, one week, or even one day ago does not mean they are clear of them today. Further, there have been reports of bedbugs on transit, in stores, office buildings, and movie theatres .... any place warm where people and their belongings are. Clearly they are not a problem that is going to go away anytime soon.

Google yourself some information on detection, and protect yourself, not only from staying in an infested accommodation, but perhaps more importantly, from taking them home with you.

For instance, besides checking over the hotel room bed and environs for signs of bedbugs, never, ever put your suitcase on the bed - use the suitcase rack (pulled away from the wall). When you get home, don't take your suitcase up to your bedroom to unpack. If it is cold (below freezing) when you get home, you can upack any liquids and leave your suitcase outside (in the car or out in the garage) overnight.

New councillor calls for repeal of DDT ban to fight bed bugs

A new Hamilton city councillor says he wants to ask the province to allow the use ofbanned chemicals including DDT to help rid the city of its bed bug epidemic.

Matthew Green, who was elected in October to represent Ward 3, says he wants the province to take a closer look at repealing the 40-year banon DDT, or other powerful chemicals on a limited basis if they'll help eradicate bed bugs in Hamilton.

Drastic measures are required, he said. While campaigning, he saw multi-unit buildingswhere upwards of 70 per cent of residents had suffered from bed bugs. Some of them were CityHousing Hamilton buildings.

I need to take a closer look at the science, but there are chemical solutions and Id like to revisit that, he said. Itd have to be evidence based.

The scientificevidence may be against such an idea, at least for DDT. Canada banned the chemical in 1972 because of its impact on humans and wildlife.

DDT was a common pesticide but had a particularlydamaging effect on birds, causing a thinning of the eggshells and damaging their reproductive abilities. Populations of Peregrine Falcons, eagles and other birds were harmed by the chemical. It's impact on bird populations was chronicled in Rachel Carson's classic book, Silent Spring.

On its website, the federal government lists it as moderately toxic to humans, with low-to-moderate level impacts including nausea, diarrhea and eye irritation. Human deaths from exposure to DDT are rare.

The idea of using DDT for bed bugs is a probably non-starter, said Chris Darling, a senior curator of entomology at the Royal Ontario Museum. I dont think thats going anywhere.

Green knows that some of these chemicals have potential health impacts. Butthe mass scale of bed bug infestations are causing health impacts too, he said.

Its drastic and its something were going to have to look at.

A brother of a man who died while living in a home run by People Helping People, a neighbour who gave food to residents of another home, and a workergive an inside look at the organization under scrutiny after a fire Monday that led to a mans death.

Kevin McNamee died alone, in a basement room on Fleming Dr. in Londons east end.

The 51-year-old had been in that locked room alone for about three days by the time another resident, who like Kevin had mental health issues, knocked down the door and found him Oct. 22, 2012.

Im horrified. I trusted (the organization) with my brother. Its been a big loss to me, said Robert McNamee, Kevins brother.

The organization caring for Kevin was People Helping People, now under public scrutiny after a fire Monday that killed a man in an apartment building on Oxford St. E.

The organization is run by Keith Charles, who says he is a former drug addict who started People Helping People about six years ago after noticing those with addictions and mental health issues had nowhere to go after being discharged from hospital.

The McNamees story is just one that The Free Press has heard since the Monday blaze, raising questions about the need for better housing for the mentally ill, and better oversight at unregulated homes that promise hot meals, help with taking medication and round-the-clock supervision.

Reached Thursday night, Charles said he remembers Kevin but could not answer any questions because he was standing in the rain and he needed to help residents of the burned-out Oxford St. building.

When Kevin was discharged from Regional Mental Health London in August 2011, he had nowhere to go, his brother said. When Charles approached the brothers in the hallway of the hospital about a place to live with hot food and supervision thrown in, Robert McNamee said he was thrilled.
